Having trouble with Scott Bradley's form

I'm wondering if anyone can point me in the right direction here. I
have downloaded the survey form template and customized it to display
almost properly. I'm not sure if I should be posting this online, or if
it is OK to send to your email, but when I look at that thread, the
last post is quite a while ago.

In the question box of this form, after the user gets it, it cites the
question followed by the choices like this:
Test question {choice1;choice2;choice3;choice4;choice5}, the lists
these choices as planned in the answer box below.
How do I get rid of the {choice1...} in the question box? It looks
cluttered with this here.
I also want to be able to format the font characteristics on the
question box.

To get rid of the {Choice1...} displaying, I've attempted to remove the
" {" & txtchoices & "}" from line 72, but that only seems to change the
answers with the radio buttons in the "frachoice" answer box to be the
questions themselves.

My biggest problem is really to compile the data. I am not a
programmer, but I am trying to learn. Sue Mosher had mentioned to copy
and paste from a table view to Excel.

I really don't understand how to have a table view include the text in
the body of the email. Somehow I need to get the data from the body of
the emails to an excel spreadsheet. I've looked at the
Accumulation/reporting script, but it is beyond me right now, how to
install it to Outlook 2003.

Any help is appreciated.
