Header disappears when document is printed

We have encounted a strange problem with one of our word 2007 files.
It is a 4 page document, and when we print it, the header does not print on
page 3 (header is simply blank). If we do a print preview, there is no
problem, and in editing there is no problem either.
We have tried to print the document on several machines and on different
printers, but the result is always the same.

Is that header different from the other pages?

If you save to a pdf file, does the header show up in the .pdf okay?

If the latter happens, then it might be a printer driver problem. What
printer are you using? You might check the printer mfg's web site and see if
there is an updated driver available.

It's the same header on every page. I suspect something might have corrupted
the document, so i'll try to recreate it.
