Header / Footer Auto Text Selection are missing.

I am using Microsoft Office 2003 and recently noticed that the autotext
options of the header/footer control are missing. I am talking about the
dropdown control on the left side of the header/footer control bar that has
options like file names, author, page x of y, etc.
All I am getting is a blank field with no entries.
The question is how do I fix this?

The usual reason for this is that you have saved a document in or as
Normal.dot. See http://word.mvps.org/FAQs/AppErrors/BlankDocNotBlank.htm
