Header for a mail merge document

Hello,
I want to define a header for the second page onward in a document. The
document is the main form of a mail merge document and it is one page long
at definition. When the mail merge is executed, the document becomes multi
page. How can I define the header for the second page onward in this
situation. I would appreciate any help.
