Headers/Footers won't enable

I have a word (word 2003) document which is having header/footer
problems. I was asked to look at it by a teammate who says the
headers/footers keeping "dropping". It is not the white space as I am
familiar with how to expand that. The headers/footers truly seem to
have disappear. When I click View -> Header and Footer it takes me to
the top of my page but the header/footer editting boxes haven't
appeared. What could be causing this?

What are the settings for "From edge" and margins in the Page Setup dialog
box?
