Yes - it's a little painful, but possible.

On the bottom of the page preceding the "form" on which
you do not want a header or footer, or page number,
select Insert/Break/Next Page to create an artifical
section break.

At the top of the page where you do not want the header,
double click on the header and select the icon from the
task bar that asks if you want to delete this header and
connect to the previous section - select NO; then delete
the header.

Move down to the footer on the page where you wish to
delete it; double click on the footer, and again, select
the icon from the task bar that asks if you want to delete
this footer and connet to the previous section - select
NO; then delete the footer.

This is so confusing - when you select NO - you are
telling the software that you actually don't want to
connect to the previous section, that you either want to
change it to a new header/footer, or want to delete the
existing text and leave it blank - the latter case is what
you are after - to leave it blank.

Now, if you have additional pages following this form, you
will need to reset the header and footer. To do this, at
the bottom of your FORM, that has no current header,
footer, or page number, you need to again
Insert/Header/Footer, then double click on the header and
go back 2 pages to copy the former header, then paste it
on the new page.

Do the same for the footer - and reinsert the page number
by selecting option and Continue from previous section or
restart at and the appropriate number.
