Help creating a "journal" in Word

I was looking to see if anyone could help me with making a journal-style document. I am looking for help with these things:

1. - Putting a custom TOC that includes custom fields (not only document page number and title, but a defined textbook page number and entry start date as well)
2. - A header for each entry that includes the title, the textbook page number and the start date.

Could anyone help me with this?
