I am trying to create a form that teachers can use to report discipline
problems to me (the principal). The form needs a place for the students
basic demographic information and a space for the teacher to type a short
narrative of the problem and a way for me to reply with some sort of outcome
(short narrative).
I have been experimenting with designing a new outlook form, I have also
used a word form and sent by email, and I have played with creating an Adobe
pdf interactive form. They all seem to have their different problems. I
need to know what a professional computer person would use.
It (the individual incident) also has to be able to be save in a teachers
file on his/her pc, and my pc. Thanks I hope I am on the right track or is
it something that requires and Access database???

I've always found that a good way to sort out such projects is to think about inputs and outputs. It sounds like you've explored the various input options fairly well, but have you though about the output side? What do you want to be able to do with the information that the form gathers? Think of as many possible scenarios as possible, and let us know.

If you find yourself inclined toward an Outlook form, a critical issue that will affect its effectiveness is your mail environment (client and server). So you'll need to provide that information as well.

I am going to use the form as a way to quickly convey to me a discipline
offense committed by a student. If it is electronically transmitted to me
from the teacher that takes out the possibility of the child loosing it on
the way to the office or throwing it away. It also conserves paper. It also
has to be able to be saved to a folder by the teacher for her/his records.
And saved to a folder by myself for my records. When I recieve the form from
the teacher via email I must be able to read the offense, write a reply, send
it back to the teacher, and file it in a folder on my pc.
It would be nice if it would be able to autocomplete with basic demographic
information , but that isn't necessary and I do not know if it outlook could
do a small "database" of about 250-300 student names and address information.

I really just need a form/template that is easily mailed to another party
and back and able to be filed.

Thanks again, but could you tell me what I should be looking for when you say
mail client and server environment. I know we have an outlook web based mail
system. It can also be viewed through outlook 2002 with a little
configuration. About the server I do not know the whole school system run
through something and each school has an individual server. What words or
numbers should I as a tech person for?
