Help:Inserting an Index

I am having trouble 'inserting' an index. Please help.

I have 'marked' all the words I want indexed . . . but I
can't figure out how to 'insert' the index at the end of
the text.

When I select 'insert' and then select 'tables & indexes' -
-- the 'okay' button is in shadow and can't be activated.

Please help. Email me off-newsgroup please!
