HELP! My e-mails keep duplicating when I reply or forward them.

When I reply to e-mails they are sent to my outbox, inbox,etc. Also they keep
duplicating i.e. I replied to a particular e-mail 5 times (back and forth)
and it ends up putting it in my outlook inbox & outbox EACH TIME! how do I
fix this? Any replies would be GREATLY appreciated!
