Help (normalisation)

[Jaffa]

Hi everyone.

I'm taking an IT exam shortly and I need to know how to do normalisation.

I'm not sure if this is the correct place to post, but I thought I'd give it
a go. So if anyone can help, the following is the data I'm attempting to
normalise, I've managed to progress through with it to third normalisation
form but I've no way of checking to see if it's correct.
