HELP: Outlook 2003 update and LDAP popup??????

I just upgraded to Office 2003 including Outlook. When I star the ap, I get
a popup (looks like input form) with a header reading "Microsoft LDAP
Directory". It displays line items for Server with a value of null; Port
with a value of 3268; User with a value of null..

What's causing this and how do I get rid of it?

You need to remove the ldap/active directory address book from your profile.
Try Tool | Email Accounts | view/change address books (4th radio button i
believe) | highlight the ldap address book and select remove.
