HELP!! printing a custom form in OL2K

I just resently made a custom appointment form in OL<2K.
I added the XPrint control to the form to print it out as
you see it. I used the same script as provided by
Microsoft (Microsoft Knowledge Base Article - 238778
OL2000: General Information About the XPrint Control and
Add-in) except I used "P.2" instead of "General". There
is no "General form" in the Appointment form. The problem
I'm having though, is when I go to print it, OL crashes.
This is the error I get:
AppName: outlook.exe AppVer: 9.0.0.6604 ModName:
fm20.dll ModVer: 2.1.9922.1 Offset: 00013eab

I do have combo boxes in the form. I also have Service
Patch 3 installed.

Any suggestions?
