Help w/ accessing SQL from form

Hello,

I would like to create a button that clicking on the button will give me a
list of SQL Servers. Preferrebly with a list of their databases as well.
Once the server is clicked then the path is placed in a field for everyone
to see. Is this possible? If so does anyone know where I can go look?
