Help With Cell Merging maybe....

I'm trying to make a sign-in sheet with a title on top
and centered. I merged the cells on that line, and put
the title.

Then I put info in 6 different columns. I wanted to print
the form, but when I clicked File, the Title disappeared.
I tried it a few more times. I don't know what I'm doing
wrong. I've merged cells before making CD inserts. It
worked and printed fine. Can someone tell me what I might
be doing wrong?
