Help With Custom Form

Is it possible in Word 2003 to create a form with 3 distinct fields of
various sizes that can have either and image, text or image and text
inserted? I need the fields to be set sizes that do not change in
shape or size when the image and/or text is inserted. Also, when the
form is printed the outline of the shape (text/image placeholder)
should not print. I'm struggling to determine if this is possible.

It is possible but you may have to use more controls than what you are
describing. I am not sure how to put text in a picture but you could
always have a label and a picture and then choose to use them if they
are populated or not.
