Help with Data Entry Form

I have to create a database form that will allow our HR personnel to
keep track of employees time off. I need to create a form that will
contain the following information.

Dropdown: Employee Last Name, First Name, Employee ID
Textbox: Date request put in
Textbox: Date they are taking off
Dropdown: Type of time off (Vacation, Sicktime, Medical Leave..etc)
Textbox: Hours taken off

What I need is a way for her to enter this data from a timesheet and to
be able to record this information and move onto the the next employee.
I need her to be able to tab down to a button that will enter this
data and clear the form and allow her to enter a new record.

Can someone tell me how to create a form to do so?

How do I create a single page form specifically for this data entry? I
don't want to be able to pull up any previous records using the form,
just enter new records.
