help with forms!!!PLEASE

I really really need some help with my form. I made a form with MS
Frontpage 2000. I followed some directions that I had from a book called
"Teach yourself visually, Frontpage 2000."
But my form does not work, and at the top of my form, it says "[FrontPage
Save Results Component]"
I need to make that go away asap, and need help with my form.
If anyone can take a look at the form for me, that would be so great. the
address is http://www.mcclain-realty.com/marketanalysis.htm.
I am new to the forms part. I have a cgi bin on my server, and my "publish
to web" feature is all messed up, so I am using cute ftp to load my pages
one at a time.
anyway, any help would be greatly appreciated.

How did you upload the form? Unless you publish the web, this will happen.

The FrontPage forms component requires server extension support. If you have
server extension support you should not be FTPing and may need to contact
your host to re-install them.
