Help with removing an add in

Help - long...
I had winfax pro 10.02 (latest version) on Windows 2000 Pro with all
updates.
I installed Outlook 2003 and enabled the add in.
Winfax has been buggy for me and I decided to uninstall. Unfortionately,
during add/remove, it said it could not find the uninstall file (log). I
tried to reinstall, only to have the install (same one) cd say it found an
earlier version of winfax and to uninstall it first (arghhh), then exited
the install process.
I manually deleted (I know) the program, used a registry cleaner to remove
all (I think) notes in the registry, ran services.msc to prevent any thing
trying to start up.

Here is the problem...
When I start Outlook, I get an error box that pops up saying,
"c:\documentsandsettings\asamuels\applicationdata\microsoft\addins\wfoladin.
dll is not a valid office add in"
I hit "OK", then Outlook continues to load.
I have viewing of hidden files enabled. When I go to that folder, there is
no such file. When I do a search for that file, it is not found. When I go
to add-ins in Outlook, it is not in the add-in box.
Any ideas on how to get rid of this error?

I took the serious step of regedit.
Backed up first, then searched for wfoladin. Several instances, deleted them
all.
It is no GONE.
Whew.
