Help with report from query design

I have a database table with 4 'Follow Up' dates - 1Week 1Month 6Months 1Year
- and every record has a date in each field. I want a date range report
displaying/grouping together names of those who had a 1Week follow up within
the date range, then group together names of those who had a 1Month follow up
within the same date range, etc. Problem: I get the correct records,
however, I also get all 4 of the dates in the record. If possible, how do I
display only the names and dates in the date range?
