Help with setting up a template

I am trying to set up a Word document to automate generating and
printing certificates. The certificates each have a customer name and
unique certificate number on them and there are 6 cetificates per
page.

Is there a way that, upon opening the document or initiated by some
other prompt, I could enter the customer name and Word would insert
that name in the correct place and increment the certificate numbers?
