hide/repress browser menus in pop up form

When a user clicks on a "Submit" link, a popup form
appears where the user can enter some information to
submit. Currently, the popup form appears with all the
browser toolbars and menus at the top (i.e., it's a full
webpage). How do I edit the popup form so that the
browser toolbars and menus don't appear?

Sorry. Posted too soon.

I suppressed the toolbars, etc. by changing
the "toolbar=yes,location=yes,scrollbars=yes,menubar=yes"
attributes to "no."
