Hotspot Form

I've been trying to figure out how to add a form to a hotspot. A small
graphic (a shopping bag), as part of a larger one, needs to actually go to
PayPal to list the items that are in the "bag." I know how to add the form
with a small separate image, but can't figure out how to do it on an image
map. Any help would be much appreciated. Thank you.

Slice the larger image & stitch the form button into it in a table
