How can I create an online survey?

Hi, I'm a graduate assistant in Management Information Systems (MIS) at
Roosevelt University in Chicago. My professor asked me to set up an online
survey of alumni to determine trends in the MIS field, relevancy of the
curriculum, where they are working, and viability of an undergraduate
program. The university has a web site, as does the professor. I am familiar
with most of the Office suite, including Word, Access, Excel, Outlook,
PowerPoint, and Publisher. I am getting familiar with FrontPage, which is the
program used to design most of the pages on the university site. I also have
experience with HTML, VB.NET, and have used the save as a Web page feature in
the Office applications. However, I have never created a Web page myself. Can
anyone out there offer assistance, advice, tips, etc. in our effort?

Open frontpage and go. If you can use office, it is very similar. There may
be a more sophisticated way to create an online survey, but I have used a
Frontpage form. (From the Help file): On the File menu, point to New, and
then click Page or Web.
In the New Page or Web task pane, under New from template, click Page
templates.

Hi,
Thanks, I read that in the MS Office Online tutorial on creating forms as
well. It sounds like a good solution, but I'd like to keep some consistency
with other pages on the site. When I try to save a university page as a web
page and then access the file in FrontPage, some of the graphics and links
don't show up. I guess this could be because those links are on the school's
server. Any other ideas?

If you mean you start with one of their pages, save it to your own computer
and cant see the graphics, you are correct that it is because the graphic is
not your local web. If you double click on one of the missing graphics, and
select the second tab, and then browse, it will show you where it thinks the
image file is. When you publish your page back to the server, the location
of the graphic (picture properties) might need to be reconfigured, depending
