How can I create outlook survey like word template

How can I use functionality similar to Word template, to design a form with
checkboxes, drop downs, fields...but to do that in the body of an email
(meaning no Word attachment)? I want the user to see the survey in their
email and respond directly by replying.

You should be able to create the template in word with all of the form fields
and then click on e-mail icon on the standard toolbar. This will bring up
the familiar e-mail address screen.

Thanks - tried that but when the email is received, none of the fields or
checkboxes appear. The recipient gets just the text so the template
functionality is not there. Other ideas?
