How can I embed a form to gather contact info

I would like to find out how to embed a form into an email message to gather
contact information from a distribution group. I have about 150 members in
our Scouts group and have only partial contact information. What I would like
to do is send them all an email requesting that they fill out a "Contact"
form with all their data.

How can I add the form to the email so that they can fill it out and reply
back to me and get their information so I can import it into Outlook.
