How can I find a template for an offical notary form?

I had to have some documents (letters) notarized to verify my signature. The
notary public person at my bank said that it was not in the correct form.
What is the correct notary format?

Anything can be notarized. Ask the bank teller what forms he/she thinks
you're supposed to have.
