How can I fix my Frontpage guestbook?

On Microsoft Frontpage 2003 I am using their guestbook page on my website.
But when someone trys to sign it, it says the following
---------------------------------------------------------------------------------
FrontPage Error.
User: please report details to this site's webmaster.


Webmaster: please see the server's application event log for more details.
---------------------------------------------------------------------------------

What can I do to fix this?
My website is http://www.jamesmagic.com
The location of my guestbook is http://www.jamesmagic.com/html/guestbook.htm

The guest book template does not allow additional fields to be added
- you have added a "name" field so it becomes a different form
Remove the name field and run a tool recalculate hyperlinks
<p align="left"><font face="Perpetua">Name:
<textarea name="Name" rows="1" cols="8"></textarea></font></p>

PS
Best to just create a new guestpage from the template and leave it alone - don't edit the form properties

Thanks Stefan B Rusynko, but I fixed it and the Name field works. I just had
to put guestlog.htm in the _private folder.
