How can I publish a pdf file I printed to ON and annotated?

I printed a pad document to ON, then annotated. Now I'm trying to publish it
as a pdf, Word doc or anything else so I can send it to someone. ON
publishes my notes, but not the document (image) I annotated.

I think to remember that a while ago I had something similar too.
I'm sure that it happened rather often that the resulting PDF staretd
with a page only having the title of the ON page and the rest on the
following page.

I cannot reproduce it anymore now, however :-(

P.S. How is the result when using the Acrobat printer-driver for
printing from ON?

There are 3 different ways with ON 2007

1.) From the menu "File | Publish as PDF or XMS"

2.) From the menu "File | Save as ..", choose PDF format.

3.) If you have a PDF printer driver:
Print the document select "Adobe PDF" as the printer.
