How can I specify my own image for Dialog Icons?

Hi Folks

Trying to create few software diagrams and got stuck with as to how to
extend Dialog Icons to include my own images?

Also how can I extend toolbar buttons to have my own images? I see "Blank
button" but could not figure as to how to ass my own image to it.
