How can I use Outlook Addresses in an email form?

I am trying to create an Outlook request form that will allow the user to
pick names from the Outlook Address Book, but not as a TO, CC or BCC. The
purpose is to send the form with the email names so that they can be used by
the recipient to route the signature form.
