How do I add a box that if clicked on shows a check mark?

I have been instructed to make a check list form. It has been required that
a box be placed and a user can simply click on the box and a check mark
appear and be retained. If anyone can direct me I would greatly appreciate
it. I can not find it in my Advanced XP Shelly Cashman or on line.
