How do I add a text place holder to be filled in in a template?

I need to create a template and add fields to be filled in, as seen in the
styles for headers and footers, example [Enter Text Here].

I suspect the reference to headers and footers suggests the use of content
controls, as used in many of the header/footer building blocks in Word 2007.
