How do I add my own text in every Calender entry

We have Calender to book 2 meeting rooms, about 15 other users have access. I
would like to have each new appointment to have date that I create.
This will be in a basic format

Please indicate the number and times of
Attendees:
Tea?:
Break times:
Coffee?
Break times:
Biscuits?:
Water?:
Juice?:
Cost Code:
Sandwiches?:
Bacon Rolls:

AS a basica idea every time someone creates a new appointment. At the moment
the entry is completley blank

you need to create a custom form for everyone to use when they book meetings
in that room.
