How do I auto-populate new records with data from another table?

I have a database I have created to serve as a call log that currently has no
completed records in it. I want to auto-populate three fields on the form I
am using for data entry with data from another source table. Specifically,
when a user clicks forward to a new record on the entry form I want an
identifying number, the phone number, and the associated name to appear
automatically from the other table. All I have been able to do is create a
lookup wizard that gives a drop-down from which I can select the data item,
but that allows the records to get out of order, mismatched, etc. I have
tried creating relationships but am probably doing it wrong and can not get
the data to automatically appear in a new record. Is there some way to
create the table with 5000 records with the three data fields from the source
table already loaded and then go back through them via a form to enter new
data that will constitute the new call log table? I can't get it to copy and
paste that quantity. I am using Access 2000. If someone could explain it to
me very basically I would be extremely grateful. Thanks!

How are the two tables connected? For instance, where is this phone number
coming from? The previous record, a different table, and how do you know
which phone number you want?
