how do i autofill from text box in a form to header/footer in WORD

When I fill out fields in a form, I would like to have the information
automatically put into the header. So for example, in a performance
evaluation, the evaluator would fill in the employee's name. Ideally, I
would like then for that name to be populated in the header so that it is
repeated on every page. Anyone have any ideas?
