On 2007, requested Woodall & Broome, Inc. to conduct surveillance on ,
hereafter referred to as the claimant.
SYNOPSIS
The investigation was conducted from July 2007, with physical surveillance
performed on and July 2007. During the surveillance, the claimant was
observed . These activities were performed in a normal fluid manner with no
visible signs of pain or discomfort.
Investigative efforts confirmed the claimant present at the provided address
where she remained inactive and confining the majority of hisher activities
to the interior of the residence throughout the both days of the
investigation. A canvass of the neighborhood revealed

investigation. There were minutes of recorded claimant activity. <click

INVESTIGATION
On July 2007, the investigator conducted a series of telephone inquiries
and computer research to verify the information provided by the client and to
possibly determine the claimant’s daily activities and/or employment status.
Inquiries revealed the claimant continued to reside at the file address and
had an active telephone number of () – as indicated below:
A comprehensive Internet search was also conducted utilizing a combination
of the claimant’s name, SSN and DOB. The following is the results of that
search:
No further information was obtained at this time.
On July 2007, the investigator traveled to , arriving in the vicinity of
the claimant’s residence at . Upon arrival, it was determined that the
claimant resided in a single story wood-framed home situated on a parcel of
property on a rural route. The landscaping was neatly maintained and
consistent with all other residences in the area. Upon arrival, it was noted
that the residence was situated on a parcel of property in a rural
middle-class neighborhood. No activity was noted and a stationary
surveillance position was established in the area.
Due to the location of the residence and surrounding area, the investigator
established a surveillance position in the area, which allowed the monitoring
of all vehicles entering and exiting the area.

In an effort not to arouse suspicion of neighbors, the investigator elected
to change the point of surveillance at ap.m. to a more discreet position
nearby which allowed for the monitoring of .
Telephone inquiries were conducted at ap.m., which confirmed the claimant’s
presence within hisher home.
Neighborhood inquiries were conducted at a.m. which confirmed that the
claimant was present within his home.
A drive-by check of the residence and surrounding area at , was conducted.
No change or outdoor activity was noted.
A female fitting the provided description of the claimant was first observed
as she . The sighting occurred observed during a drive-by and policies
preclude the use if videotaping of the claimant while operating a motor
vehicle; however, a photograph was obtained of the claimant as he entered the
home.
Due to heavy traffic congestion as the claimant traveled on _ in , Alabama,
the investigator lost visual contact with the claimant’s vehicle. The
investigator conducted a canvass of the area to no avail.
There continued to be no change or notable activity at or around the
residence. Stationary surveillance was relocated at to a position with an
unobstructed view of the residence.
A female in the same age group and fitting the provided description of the
claimant, hereafter referred to as the claimant, was briefly observed as she

or ap.m.. Periodic scene documentation was obtained depicting conditions at

On July 2007 the investigator returned to the vicinity of the claimant’s
residence arriving at ap.m. Conditions associated with the residence,
vehicles and surrounding area appeared unchanged from the previous date of
surveillance. No outside activity was noted and a stationary surveillance
position was established in the area.

In an effort not to arouse suspicion of neighbors, the investigator elected
to change the point of surveillance to a

aggressive but still discreet surveillance position was obtained in an
attempt at monitoring the claimant should the claimant become active in his
yard.
Due to the lack of any claimant activity, surveillance was discontinued at
p.m.

discontinued at p.m.
A neighborhood canvass was conducted at p.m. in an effort to obtain
information regarding the claimant’s current activities, physical health
appearance and employment status. The investigator arrived at and spoke, who
stated that the claimant’s residence was the last one at the end of the road
on the right hand side.
The client was later contacted and provided an update as to the results of
the investigation.
