How do I correctly address a letter to a Judge?

I need to know how to correctly address a judge in a letter. Is it "her
honor Judge so and so or what? Thanks, D.

Hardly an Office question, I would have thought but the last
pages of good dictionaries often cover that sort of thing.

OK, let's admit it, as usual you can find out with Google. Try
"ceremonious forms".
