How do I create a check box.

I need to create a 3 option check box. Each of the three
boxes needs to be linked to an import specification (as
required by the transfertext method). Checking the type_A
box should input the "type_A" specification into a
variable in the transferText method for the import
specification. The problem is this is far more advanced
that the level of VBA knowledge I have. Any help would be
appreciated.
