How do I create a document to list websites w/passwords etc?

I purchased a "pre-made" book that you can write in all the websites you go
to plus the username and your password. My problem is that certain letters
(it's done alphabetically) are already full and I have no where else to write
them in. So I wanted to create my own using Microsoft Office, but I can't
find a template that's even close to what I need, and I'm fairly new at using
Microsoft Office, so I'm having issues creating one on my own. Can anyone
help? or has anyone created one of their own and would be willing to share
their template with me?
Any suggestions?

you may have to look for a resolution
from a different angle.

have you tried a freeware called
"keypass"?

once you enter the data you can
then click on the url, then click
and drag your login data into
the webpage.

Well IE will remember passwords, other than that do you REALLY need a
template?
Just make a list in Excel - column 1 for the URL, column 2 for the username
and column 3 for the password. It's as easy as that.
