How do I create a form for use in MS Outlook?

I've been trying to create a form for use by people in the office to send me
specific data for a database. I want them to be able to use an outlook form,
but most are creating email using HTML, plain text or RTF. The administrator
has prevented us from using MS Word as our email editor.

So, what particular problems have you run into? Outlook message forms work best when you can publish them to the Organizational Forms library on the Exchange Server. Will the admin permit that?

Look about on www.slipstick.com and www.outlook-code.com for examples of the
many steps you will have to take.
