How do I create a form survey in Word?

I would like to be able to create a form survey in word and send to my
customers via email. The customer would then answer five questions (sentence
format) and submit the document back to me by clicking of an email address
embedded in the document. Each question would need to have a box or some type
of predefined area for the customer to type their responses.
