How do I create a form that can be viewed on a treo?

I have created an email message that I send to the people in my group. The
message is fairly basic except for having radio/optionbuttons, etc. However,
a few of the people have handheld devices like treos and blackberries and
they can't view the message. The only thing they see when I send them the
message is my signature. These same people have larget than the norm screens
and the form I created looks odd when they pull it up on their larger screens.

Only Outlook can be used to view an Outlook custom form. Users of other email
programs, including those on handheld devices, will see only the data in the
message body, such as your signature.
