How do I create a form, with fields, as a outlook 2000 template?

I'm attempting to create a form, with fill in fields - as a outlook 2000
template.

Ideally I would like to be able to have a outlook message template, which is
pre-loaded with a body of text that I frequently sent out.

I only change certain words in the lengthly email - This is why I have the
need for the fill in fields capability.
