How do I create a form with multi-tabs

Hi

I want to create an easy data entry form for a survey I have designed.

The survey has 5 sections: general, school, family, activities, demographics.

Instead of having one single form, I want to create a form that has 5 tabs.
One for each subject.

This needs to be a part of a larger database where information from several
areas is being collected.

How is this a "tablesdbdesign" question? You'd probably get a lot more
response if you posted in a forms-related newsgroup...
