I want to include page borders around all the text. I also want a wider
margin (or gutter) on the left in order to spiral bind on the left. How do I
set this up? When I change the margins, or change the page border to "from
text" the changes apply to all 4 sides of the box of the page borders.

Are you using the gutter setting or increasing the left margin? Either
should work provided the border is set "From text." You can increase the
distance from the margins up to 31 pts. If that doesn't work for you, you'll
need to draw a rectangle AutoShape anchored to the header.
