How do I create a New Template from Scratch

Operating System: Mac OS X 10.4 (Tiger)

I am trying to create a questionnaire and I want to create fields, but none of the templates provide on Mactopia work for me. How do I do it from scratch? Please help

from scratch? Please help

Create a new document, customize it any which way you want and when you
save, save as Template (.dotm).
You can even save it in one of the folders for custom templates for easy
access.
