How do I create a patient management database

I am a medical doctor working with about 400 HIV positive persons. I would
like to enter my patient data into MS Access. Though I have been able to
create the tables and link them, I would like some help on making forms and
queries.

Your question is much too broad to be able to answer in the newsgroups.

There could potentially be a few weeks work in revising the structure,
developing the interface, and creating the reports that you want out of this
system. You may like to talk with other doctors about what medical databases
are available for you to buy. If there is nothing suitable, then you may
like to email some of us who do this for a living to see if we can develop
something for you, or provide consultancy to guide you as you develop your
own database.

In addition to Allen's observations, I would also point out that varying
local, state/province and federal laws and regulations may govern what kind
of data may be stored, in what formats, with what security, etc., depending
on the country/location for which you are building this.

You'll need some experienced professional help if you want this
application to be compliant with the stringent and detailed HIPAA
patient privacy requirements. "Do not try this at home" is a phrase
that comes to mind...

Please look into the HIPAA requirements on the FDA's web page before
you go too far into this project; you could be laying yourself open to
some serious legal liability.
