how do i create a questionnaire to send to users

I have been asked if there is a way to create a questionnaire in Outlook.
The user wants to send out a marketing questionnaire and receive the
confidential responses. The user is particularly interest to find out if
outlook can total the responses.

If this requires VBA, can you send me in the right direction?
