How do I create new icons for the QuickAccessToolbar in Word2007?

I was able to create icons for convenience macros that I use in Word 97, but
can't seem to do so for Word 2007. Does anyone know how to do it. If not, why
has this excellent feature been removed? (I ask not why is the UI of this new
Word so counter-intuitive that after using it for a month, I am still
searching all over for the tools that I need).
